Book Description: A delayed discharge occurs when a patient is ready for transfer from a general and acute hospital bed but still occupies that bed. It used to be called bed blocking. This report looks at both the hospital issues and the external issues such as access to rehabilitation and other care facilities, inter-agency co-operation, the management of alternatives to hospital admissions, communications and the impact on patients, staff and carers. It is organised in three sections. The first lists the main policy measures the Government has put in place. The second section analyses, chronologically, the processes involved in delayed discharges and mirrors a patient's journey through the hospital and care system. The final section looks at the structural issues relating to the health and social care system.
